Ongoing care for long-term conditions

Chronic conditions need regular attention, not only treatment when you feel unwell. Dr Omar provides ongoing care at Riverside Mall, Rondebosch, with a plan that fits the individual patient.

Conditions we manage

  • Diabetes and hypertension. Assessment of diet, activity levels and medication, and monitoring for damage to organs such as the eyes, kidneys and feet. Patients are managed in a care programme.
  • HIV. Management and monitoring, with the aim that patients live well on treatment.
  • Asthma, eczema and allergic rhinitis. See allergy management.
  • Other long-term conditions. Ask us whether we can help with your condition.

What a chronic review usually involves

  1. A discussion of how you have been, including symptoms and any side effects.
  2. Measurements such as blood pressure and weight, and tests when they are indicated.
  3. A review of your medicines.
  4. Advice on diet, activity and other lifestyle factors.
  5. Checks for complications and a plan for your next review.
Bring all your current medicines or a list of them, and your recent test results, to your first chronic care appointment.

Medical aid chronic benefits

Many medical aids pay for chronic medicine when a condition is registered on a chronic benefit. Requirements differ between schemes. Ask us what your scheme needs when you book. Read more in our article on ongoing care for high blood pressure and diabetes.

Frequently asked questions

How often will I need to come in for a chronic review?

It depends on your condition and how well it is controlled. Dr Omar will tell you how often he recommends a review.

Can reviews be done by phone or video?

Some follow-ups can be done by telephone or video when it is clinically appropriate. You may still need in-person checks such as blood pressure and blood tests.

Can I get repeat prescriptions?

Ask us about repeat prescriptions for stable conditions. The doctor must be satisfied that it is safe to continue your treatment.

Does my medical aid cover chronic medicine?

Many schemes cover chronic medicine for registered conditions. Ask us what your scheme needs, as some require an application for chronic benefits.
